1. General Information
• These Terms and Conditions govern the services provided by Vaxcen Corporation (hereafter referred to as "the Center") for the administration of vaccines to individuals (hereafter referred to as "the Patient"). By scheduling an appointment or receiving services from the Center, the Patient agrees to abide by the terms outlined here.
2. Appointment Scheduling
• Appointments can be scheduled through the Center's website, phone line, or in person.
• Appointments are subject to availability, and the Center reserves the right to modify or cancel appointments as needed. Patients will be notified of any changes.
• The Patient must provide accurate and up-to-date personal and medical information during the booking process to ensure the proper vaccine is administered.
3. Vaccine Availability
• The Center offers a range of vaccines, including but not limited to COVID-19, flu, travel-related, and other immunizations, depending on availability and local regulations.
• Vaccine availability is subject to supply and government regulations. The Center is not responsible for any delays or shortages in vaccine stock.
4. Eligibility for Vaccination
• The Center may require the Patient to complete a pre-vaccination screening questionnaire to assess eligibility for the vaccine.
• Some vaccines may have specific age, health, or medical condition requirements. The Center reserves the right to refuse vaccination if the individual does not meet the criteria for a particular vaccine.
• If the Patient is under 18 years of age, a legal guardian must provide consent in writing.
5. Consent for Vaccination
• By receiving a vaccine at the Center, the Patient confirms that they have read, understood, and consented to the information regarding the vaccine and potential side effects.
• The Patient acknowledges that they have been given the opportunity to ask questions about the vaccine and any potential risks.
6. Payment
• Payment for vaccines may be required at the time of service, unless covered by insurance.
• The Center accepts various forms of payment, including credit/debit cards, insurance claims, and other authorized methods.
• Any outstanding payments must be settled before the vaccine is administered.
7. Insurance
• If applicable, the Patient should provide insurance details at the time of scheduling or upon arrival.
• The Center will submit claims to insurance companies, but the Patient is responsible for any remaining balance, co-pays, or out-of-pocket expenses not covered by insurance.
8. Side Effects and Aftercare
• Vaccines may cause mild side effects such as soreness at the injection site, fever, or fatigue. These side effects typically resolve within a few days.
• If the Patient experiences any severe side effects (e.g., difficulty breathing, swelling), they should seek immediate medical attention and inform the Center.
• The Center will provide instructions on post-vaccination care and may request the Patient to wait for a specified period after receiving the vaccine to monitor for any immediate reactions.
9. Privacy and Confidentiality
• The Center is committed to protecting the privacy and confidentiality of the Patient's personal and medical information.
• Personal data will be stored securely and will only be used in accordance with applicable laws and regulations regarding healthcare privacy.
10. Liability and Waiver
• The Center is not liable for any injury, illness, or side effects that occur as a result of receiving the vaccine, except in cases of gross negligence or misconduct.
• The Patient agrees to release the Center, its employees, and agents from any claims or liability arising from the administration of the vaccine, except as prohibited by law.
11. Cancellation and No-Show Policy
• If the Patient needs to cancel or reschedule an appointment, they should notify the Center at least Three(3) hours in advance.
• A "no-show" or failure to attend an appointment without prior notice may result in a cancellation fee or the inability to reschedule.
12. Changes to Terms and Conditions
• The Center reserves the right to modify these Terms and Conditions at any time. Any changes will be communicated to the Patient through appropriate means.
• It is the Patient’s responsibility to review the Terms and Conditions regularly.
13. Governing Law
• These Terms and Conditions shall be governed by and construed in accordance with the laws of the Republic of the Philippines.
• Any disputes arising from the use of the Center’s services will be subject to the exclusive jurisdiction of the courts of Philippine Judiciary Court.
