How To Claim?

1. To ensure the best service and vaccine availability, prior booking is required – no walk-ins accepted. Current rates are exclusively for reserved appointments. You may visit your selected branch one business day after your purchase date, upon receipt of your booking confirmation.
Note: Orders placed after 4:00 PM will be processed on the next business day (e.g., For an order placed on Monday 5:00 PM, you may visit your selected clinic as early as Wednesday morning)

2. Upon arrival, you may be asked to fill out a form provided by the clinic or laboratory. Please bring at least one valid ID.

The 5-in-1 vaccine provides protection against five serious childhood diseases in a single shot: Diphtheria, Tetanus, Pertussis (whooping cough), Polio, and Haemophilus influenzae type b (Hib).

The vaccine is administered in multiple doses according to this schedule:


First dose: 2 months of age

Second dose: 4 months of age

Third dose: 6 months of age

Fourth dose (booster): 15-18 months of age

Fifth dose (final booster): 4-6 years of age


This comprehensive vaccine helps build immunity against these potentially life-threatening diseases, with protection developing approximately two weeks after each dose.


The 5-in-1 vaccine is recommended for:

Infants and young children as part of their routine immunization schedule

Children who missed doses during infancy (catch-up schedule)


These diseases can cause serious complications, including respiratory failure, paralysis, meningitis, seizures, brain damage, and death.
